This first conversation will also help determine if I would be a good fit for your clinical needs, and if so, allow us to schedule your first consultation appointment. Our clinic uses a secure electronic medical record. For your convenience, I will instruct you on how to enter your demographic information and relevant forms prior to your first appointment on our secure online Patient Portal.


What do I need to do before this consultation appointment?

Why the Patient Portal?

Information provided in the Patient Portal will be reviewed prior to your first visit. Consider what your goals of treatment are. We will discuss them during the consultation visit.


What do I need to bring?


Evaluations consist of at least one office visit, an extensive history review, as well as a review of relevant documentation. Time spent obtaining information and coordinating care with your other clinicians, medical providers, and/or school contacts, will be billed at the full session rate whenever appropriate. Coordinating care with your other providers helps me to gain a better understanding of the issues leading to the consultation, and will allow me to formulate the best and most appropriate treatment plan for you, your family, or your child.

At the end of the consultation process, I will provide you feedback and appropriate treatment recommendations. Treatment recommendations can include psycho-pharmacology; various modalities in individual psychotherapy, organizational skills training, and/or parent management training. Treatment may include working together or providing referrals, whenever appropriate, to provide the best care.


ADULTS

Initial evaluation sessions for adults are generally 90min in length, potentially followed by one to two additional sessions (up to 45min in length) depending on how much ground there is to cover, before we meet to discuss recommendations for treatment.


CHILDREN AND ADOLESCENTS

Evaluations for children and adolescents generally consist of at least two 90-minute appointments.  The initial appointment is with the parents or family, potentially followed by one to two additional sessions (up to 45min in length), depending on how much ground there is to cover. The process is repeated with the child or adolescent. An additional session will then be scheduled to discuss feedback and treatment planning.

I aim to provide the best care possible to you, but please be advised that the evaluation process takes time and I am not able to accommodate urgent appointment requests. If you ever have an emergency, please call 911 or call the National Suicide Prevention Lifeline at 800.273.8255.


PRIVACY

Your privacy is always of the utmost importance to me and I will not release your information without obtaining a written consent.  All your personal information and records are kept secure and confidential.  The only situation where I may be required to compromise your confidentiality is in the case of an emergency, a situation of imminent danger to your life or the life of another person.
